Postnatal Consultation
A typical lactation consultation involves both visual and physical assessment of the baby's mouth and the mother's breasts, observing the baby's breastfeeding technique, reviewing breastfeeding history, demonstrating methods to enhance feeding, and occasionally using specialised equipment.
I consent to Lactation Consultant Jill Gibson, Breastfeeding Specialist, performing these services.
I recognise that I may refuse any recommended techniques, equipment, or actions at any time.
All medical care will continue to be managed by my doctor and other healthcare professionals, with permission for relevant information to be shared with them as necessary. If there are concerns regarding the welfare of myself or my baby, Jill Gibson will share relevant details with other professionals as required.
Antenatal Consultation
During an antenatal lactation consultation, you can address any worries, discuss strategies for successful early breastfeeding, and make a personalised plan to boost confidence and preparedness. Examining the mother's breasts can support this process.
I give permission for Jill Gibson, Breastfeeding Specialist, to provide these services.
I understand that I may choose not to participate in any suggested actions or recommendations.
Ongoing medical treatment will remain the responsibility of my doctor and other health professionals, with consent for information sharing when appropriate. Should any concern arise about the wellbeing of the client or baby, Jill Gibson will involve relevant professionals.
Record Keeping
A record containing personal and medical details will be created during your consultation and securely stored by Breastfeeding Specialist services, adhering to the NHS Medical Records Code of Practice (Scotland) 2012. You have the right to request access to this record at any time.
Your contact information will be securely retained according to General Data Protection Regulations (May 2018), exclusively for use by Breastfeeding Specialist services and never shared with third parties.
Please note: Clinical notes for babies' care must be kept for 25 years, as required by NHS Code of Practice (Scotland) 2012.
Breastfeeding Specialist services is registered with the Information Commissioner’s Office and ensures all information is securely managed according to the NHS Code of Practice Scotland (2012).
